When those last people put me through an entire appointment that didn't involve taking impressions of my teeth, but did end with a pitch for a $4,000 out-of-pocket TMD treatment program, I called them assholes on my way out. I also sent them a letter requesting a refund for the cost of the visit. I've totally lost patience with this kind of shit.
