On an Azure VM with Accelerated Networking, the kernel oopsed in the hv_pci eject
worker when the host removed the SR-IOV VF (presumably during host servicing). The
worker exited with IRQs disabled, the host re-offered the VF about a minute later but
the guest never probed it again, and from then on the guest could not make outbound
